چکیده

1 Assistant Professor of Orthopaedics, Associate Director of Rehabilitation, University of Miami School of Medicine, Miami, Fla. 33152. 2 Director, Prosthetic-Orthotic Center, Northwestern University Medical Center, Chicago, Ill. 60611. 3 Orthotic Division, Indiana University Medical Center, Indianapolis, Ind. 46207. There is liittle question that the field of orthotics has taken a back seat to prosthetics in modern times, and perhaps for good reason. The needs of the amputee are more immediate and obvious, and the wars of the past thirty years have yielded untold numbers of young men in their prime whose productivity depended upon satisfactory functional restoration of their missing limbs. Medicine, engineering, and the prosthetic profession have responded to the needs of the amputee through extensive research and development, widespread educational programs, improved fabrication and fitting techniques, and better delivery of services. The field of orthotics remains in comparative disarray with more limited, though no less sophisticated, research activities, few educational endeavors, and little improvement upon local fabrication and delivery services over the past fifty years.

'Trial and error…', '…happy patients' and '…an old toy in the cupboard': a qualitative investigation of factors that influence practitioners in their prescription of foot orthoses.

BACKGROUND Foot orthoses are used to manage of a plethora of lower limb conditions. However, whilst the theoretical foundations might be relatively consistent, actual practices and therefore the experience of patients is likely to be less so. The factors that affect the prescription decisions that practitioners make about individual patients is unknown and hence the way in which clinical experi...

Influence of a custom foot orthotic intervention on lower extremity dynamics in healthy runners.

OBJECTIVE To investigate the influence of a custom foot orthotic intervention on the lower extremity dynamics in healthy runners. DESIGN Three-dimensional kinematics and kinetics were collected on 15 female runners (>15 miles per week) while each performed the over-ground running trials in either a shoe only or a shoe+custom foot orthotic condition. Kinematic and kinetic variables were analyz...

Use of anti-pronation taping to assess suitability of orthotic prescription: case report.

This case report describes a strategy for assessing the suitability of orthotic prescription for individual patients with lower limb overuse injuries. The case concerns a 32 year old male soccer player with a two-year history of Achilles tendinopathy. A functional assessment performed before, during, and after a trial period of anti-pronation taping showed that taping reduced symptoms markedly ...

Foot orthotics in the treatment of lower limb conditions: a musculoskeletal physiotherapy perspective.

Orthotic therapy is frequently advocated for the treatment of musculoskeletal pain and injury of the lower limb. The clinical efficacy, mechanical effects, and underlying mechanism of the action of foot orthotics has not been conclusively determined making it difficult for practitioners to agree on a reliable and valid clinical approach to their application and indeed even their fabrication. Th...

The influence of a foot orthotic on lower extremity transverse plane kinematics in collegiate female athletes with pes planus.

Non-contact anterior cruciate ligament (ACL) injuries in female athletes remain prevalent. Athletes with excessive foot pronation have been identified to be at greater risk for non-contact ACL injury. Excessive foot pronation has been linked to increased medial tibial rotation. Increased medial tibial rotation heightens ACL strain and has been observed at or near the time of ACL injury. Foot or...
